Touch To Code-Neurology follows the documentation requirements for the specialty of Neurology as specified in the document "Documentation Guidelines for Evaluation and Management Services" published in May, 1997 and accessible from within Touch To Code. It is not applicable to other specialties or to the earlier Guidelines published in 1995.
Touch To Code is based on the point system that many CMS contractors use to assess the relationship between the medical record and the CPT code submitted for reimbursement, but it is not a substitute for a thorough understanding of the documentation requirements.
